The 2019 Community Health Needs Assessment provides insight into our service area, including, but not limited to, demographics, unemployment rates, educational achievements and access to care over the past few years. It also presents a few of Aspire’s many healthcare services and the strategic approaches that have developed in order to provide holistic care. 


This Community Health Needs Assessment has been completed for strategic planning, the benefit of those we serve, and to comply with the Health Resources and Services Administration’s requirement that a community health needs assessment be provided, at a minimum, once every three years.

Federally Qualified Health Center (FQHC) is?

A Federally Qualified Health Center (FQHC) strives to help meet the needs of an underserved area of population. For over 50 years, these government-funded health centers have delivered high quality, affordable and accessible primary care services to the community at large. Today, one in 12 Americans rely on an FQHC for health care services. 


Aspire Indiana Health was awarded FQHC Look-Alike status effective September 1, 2017. We are committed to treat underserved, underinsured, and uninsured Americans.
