Infectious Disease Program

  • Identify clients’ needs while developing a goal-oriented care plan to meet the needs of those living with HIV/AIDS and/or Hepatitis C or at increased risk for contracting HIV/AIDS and/or Hepatitis C.
  • The case management and navigation programs consist of goal-oriented activities that serve to locate, facilitate access to, and monitor the full range of HIV-related or HCV-related services in cooperation with the client. It encourages the most cost effective use of medical and community resources, and promotes the overall well-being of the individual.
  • We respect cultural diversity, emphasize confidentiality, and strive to ensure the clients’ freedom of choice and self-determination. 
  • Our comprehensive and compassionate services are rendered in a safe, secure, and non-judgmental environment.
  • These services are provided in 32 Indiana Counties.
  • These services are provided at no cost to the client.
Ryan White Non-Medical Case Management
  • Case Management services for people living with HIV/AIDS.
  • Provide intake and ongoing assessments to meet the changing needs of our clients.
  • Provide education to clients about the importance of maintaining a healthy lifestyle and striving for viral suppression.
  • Work with clients to identify needs and set goals for meeting these needs.
  • Coordinate necessary services to help meet the needs of clients.
  • Link clients with HIV-related financial resources (Emergency Financial Assistance, Housing, Food/Nutrition Support, etc.)
Housing Opportunities for Persons with AIDS (HOPWA)
  • HOPWA is the only federal program dedicated to address the housing needs of people living with HIV.  Our Ryan White Non-Medical Case Managers can assist you with applying for this assistance to meet your housing needs.
  • Recipients must meet all eligibility requirements (income, residency, enrollment, and HIV status)
  • Assistance may be available for long-term rent, short-term rent, mortgage, and utility assistance.
PrEP Navigation
  • Case Management support to people with an increased risk of contracting HIV.
  • Provide education to clients about Pre-Exposure Prophylaxis (PrEP).
  • Work with clients to identify behaviors that may be putting them at risk for contracting HIV.
  • Assist clients in coordinating PrEP-related medical appointments with a provider of their choice.
  • Assist clients in accessing PrEP-related financial assistance support.
  • Provide ongoing monitoring and support for PrEP-related needs.
Hepatitis C Care Coordination
  • Case Management support to people living with Hepatitis C (HCV).
  • Provide education to clients about HCV and the importance of medical care.
  • Assist clients in coordinating HCV-related medical appointments with a provider of their choice.
  • Assist clients in accessing HCV-related prescription assistance and medical coverage.
  • Provide ongoing monitoring and support for HCV-related needs.
Harm Reduction Case Management
  • Case Management services to engage people who use drugs and have identified a need for harm reduction.
  • Services rely on client-identified harm reduction related goals in order to “meet clients where they are at."
  • Support includes assessment of needs, planning, and linking with community resources.
HIV/HCV Health Education
  • Can be provided in individual or group settings.
  • Education focus on HIV, Hepatitis C, and Sexually Transmitted Infections (STIs), including transmission risks, prevention, and where to access testing and care.
  • Can provide HIV/HCV antibody testing.
Outreach Testing
  • Provides individual HIV/HCV education (including identifying potential risk factors).
  • Conducts rapid antibody testing.
  • HIV results in <60 seconds.
  • HCV results in 20 minutes.
  • Provides referrals to PrEP Navigation, Harm Reduction Case Management, HCV Care Coordination, and Ryan White Non-Medical Case Management as appropriate.
