HIP – Healthy Indiana Plan

The Healthy Indiana Plan is a health-insurance program for qualified adults. The plan is offered by the State of Indiana, and is also known as "HIP 2.0." It pays for medical costs for members and could even provide vision and dental coverage. It also rewards members for taking better care of their health. The plan covers Hoosiers ages 19 to 64 who meet specific income levels. See below if your 2017 income qualifies.
- Individuals with annual incomes up to $16,842 may qualify.
- Couples with annual incomes up to $22,680 may qualify.
- A family of four with an annual income of $34,354 may qualify.

Keeping up-to-date beneficiary information on all of your accounts is easy to do and only takes a few minutes.
Most importantly, you can feel confident that your loved ones will receive the assets you intend for them to have.
What do I need to know?
Taking the time to choose your beneficiaries now can help your family avoid probate later. Please tell your family or friends if you've selected them as a beneficiary, because your beneficiaries must contact the plan sponsor to receive their assets.
You'll need to assign at least one beneficiary for each account; most of the time you can add, change, or delete your beneficiaries online. To get started, please provide:
- Beneficiary type (e.g., spouse, non-spouse, trust, entity)
- Full name
- Date of birth
- Social Security number (optional)
You can also name contingent beneficiaries. Contingent beneficiaries receive your assets if there is no living primary beneficiary.
On joint accounts, all owners must accept the beneficiary changes:
Note: Some retirement accounts or plans may require spousal consent before you can add or change the beneficiary. Please contact your plan sponsor if you have questions.
There are several plans that require a beneficiary like your Life Insurance, HSA Accounts, FSA Accounts, and your Retirement Accounts. Please follow these steps to update your beneficiary for each.
For your Life Insurance; Please log on to Paycom. Click on My Benefits, Dependents and Beneficiaries. Here you can add, delete, and or change your beneficiaries and the amounts allocated to them. Please remember the amount must equal 100%.
For your HSA Account; You will need to contact Star Bank and complete a Beneficiary Change Form. Please call the number on the back of you HSA Debit Card.
If you participate in the FSA Program, please logon to your account online and complete the beneficiary information. If you have any questions or need assistance contact Infinifource by calling the customer service number on the back of your card.
For your Retirement Account; You have two ways to update your beneficiary. You can complete the beneficiary form and send it to Missy Neal in Benefits or you can logon to your account and update your beneficiary online. Whichever is best for you. The form is located on the intranet under Benefits/Benefits Info/Retirment.
