HIV Care Coordination

The Program, Aspire Indiana Care Coordinators, provides case management services and identifies clients’ needs while developing a goal-oriented care plan to meet the needs of those living with HIV/AIDS.

The program consists of goal-oriented activities that serve to locate, facilitate access to, and monitor the full range of HIV-related services in cooperation with the client. It encourages the most cost effective use of medical and community resources and promotes the overall well-being of the individual.

It respects cultural diversity, emphasizes confidentiality, and strives to ensure the clients’ freedom of choice and self-determination. Its comprehensive and compassionate services are rendered in a safe, secure, and non-judgmental environment. These services are provided at no cost to the client.

 

Special Populations Support Program

The Special Populations Support Program (SPSP) strives to minimize the negative impact of substance use on the health of persons living with HIV/AIDS.

The program consists of HIV testing, referral services, outreach, education and counseling for persons with a substance use history, and referral services for those dually diagnosed with HIV and a substance related disorder.
