Health & Wellness Benefits
Eligible employees can choose between two plans: Anthem PPO or Anthem HSA (High Deductible with a Health Savings Account). The Anthem HSA plan has an out-of-pocket maximum of $6,000 for single coverage and $10,000 for family. The Anthem PPO plan has an out-of-pocket maximum of $3,000 for single coverage and $6,000 for family. The effective date of benefit is 90 days from the first day of full-time employment.
Health Savings Accounts
Employees enrolled in a High Deductible Health Plan can contribute pre-tax dollars to a Health Savings Account. Aspire funds Health Savings Accounts for eligible employees, $1,000 for employee only coverage and $2,000 for family coverage. Health Savings Account contributions for new hires are pro-rated based upon the effective date of coverage. The effective date of benefit is 90 days from the first day of full-time employment.
Dental coverage is available to eligible participants at a cost separate from medical coverage and with no deductible. In-network diagnostic and preventative services are covered at 100%. Basic and major services are covered at 50%. Orthodontic services are covered at 50% for dependent children up to age 19 which includes a $1,000 life-time maximum benefit. Maximum payment per person is $1,000 total per calendar year on basic and major services. Diagnostic & preventative services are waived from the maximum payment. The effective date of benefit is 90 days from the first day of full-time employment.
Vision coverage is available to eligible participants. Services include $10 copay on routine eye exams every 12 months, $10 copay on select lenses and $130 allowance on contact lenses every 12 months or frames every 24 months. The effective date of benefit is 90 days from the first day of full-time employment.
Prescription drugs are included in the PPO and HSA medical plans which are available at the retail pharmacy or can be utilized through the discounted Anthem Rx Direct mail order program. Aspire Indiana also provides an International mail order drug program that allows employees to receive select name brand drugs at no cost. These programs are available to eligible members enrolled in the health plan. The effective date of benefit is 90 days from the first day of full-time employment.
Anthem Health & Wellness Resources
Anthem is dedicated to your health and wellness. You may access the Anthem site to learn about your health and get support to manage ongoing health issues, or make changes in your life like trying to quit smoking, get fit, eat better and more. Anthem’s healthy tools and resources provides various health topics and health and wellness programs that include case management and health assessment. The effective date of benefit is 90 days from the first day of full-time employment.
RX Help Center
Aspire has made available to employees and their dependents who are enrolled in Aspire’s health plan access to RX Help Center. RX Help Center is a concierge prescription drug advocacy service that is designed specifically to help you with expensive name brand and specialty drugs. There is no cost to enroll in this program and the goal is to significantly lower your family’s drug costs. The effective date of benefit is 90 days from the first day of full-time employment.
Aspire Indiana CanaRx
Aspire has made available to employees and their dependents who are enrolled in Aspire’s health plan access to Aspire Indiana CanaRx prescription drug program. Aspire Indiana CanaRx is an international mail order option for name brand medications. There is no cost to participate in this program and name brand medications are obtained at no cost to enrolled members. The effective date of benefit is 90 days from the first day of full-time employment.
Aspire Indiana, Inc.’s employee insurance program qualifies for HIP Link, the state of Indiana’s new premiums-assistance program that’s part of the new Healthy Indiana Plan or “HIP 2.0.” We’re delighted to offer a new program that will help some of our employees pay their health insurance premiums and other costs. Flexible Spending Accounts
The Medical Flexible Spending Account and Limited Purpose Account can help defray out-of-pocket health, dental and vision care expenses as well as Dependent day care by allowing you to set aside pretax dollars to pay for them. By using pretax dollars payroll deductions for eligible health care and dependent day care expenses, you increase your disposable income by reducing the amount you pay in taxes. The effective date of benefit is 90 days from the first day of full-time employment.
No-cost healthcare through Aspire Indiana Health
Aspire is able to offer its employees on-site Health and Wellness Centers where medical providers are focused on maintaining and improving your health. On-site locations are available at Elwood, Anderson, Carmel, Willowbrook, and May House.

Employee Assistance Program
Confidential counseling and referrals to help you and your family manage life’s challenges. Aspire provides, through a facility other than its own, confidential counseling and referrals to help you and your family manage life’s challenges. This program is at no cost to employees and eligible dependents, and is available on the first day of employment for all staff.
We offer our employees free access to myStrength, an online app to help manage behavioral healthcare. myStrength provides evidence-based tools to improve wellness programs and help individuals overcome behavioral health issues like depression, anxiety, and addiction.
Providing family security in the event of your death is another aspect of benefits. Eligible employees currently receive Basic Life Insurance & Accidental Death and Dismemberment coverage for a death benefit equal to one times annual salary up to $200,000. Additional Life and AD&D insurance is available to purchase for employees and their dependents. The effective date of benefit is 90 days from the first day of full-time employment.
Aspire provides eligible employees Short-Term Disability benefits for an employee’s own medical condition. This benefit is utilized for disabilities from 15 days to 180 days. During the two week waiting period, employees will use available PTO, or no pay if PTO is exhausted. Benefits paid are determined by years of service. The effective date of benefit is 90 days from the first day of full-time employment.
Aspire provides eligible employees core long-term disability insurance that pays a benefit of 60% of the employee’s monthly base salary up to $8,000 per month. This plan requires a 6 month waiting period before disability benefits are payable. Employees also have the option to buy-up supplemental long-term disability insurance for an additional premium. The effective date of benefit is 90 days from the first day of full-time employment.
Payment of approved medical expenses and lost wages from an illness or injury incurred in the course of your employment in accordance with Indiana law; provided at no cost to all employees.
Aspire is committed to providing a safe and healthy workplace and promoting health and well-being of our employees and clients/consumers.
On the first day of employment, each staff member is eligible to participate in Aspire Indiana’s 403(b) Retirement Plan. Voluntary employee contributions are made through payroll deductions with pre- or post tax dollars, as elected. Beginning the first of the following quarter after completion of one (1) year of service, Aspire provides a 100% match of employee contributions up to a maximum of six percent (6%) of employee’s eligible wages. Employees must work a minimum of 1000 hours during the plan year to be eligible for the employer matching contributions.
Paid Time Off
Paid Time Off (PTO) is a benefit provided by Aspire Indiana for the purpose of providing full-time staff members with the opportunity to take time away from work without loss of compensation. The PTO benefit is an inclusive time-off program intended for use in connection with vacations, short-term injuries, illnesses, personal business, family care, school activities, emergency closings, appointments, and any other needs that may require time off from work. Payment of PTO should not be confused with payment of holiday pay, short-term disability, jury leave, or bereavement benefits. Holiday, short-term disability, jury, and bereavement benefits are each separate compensation plans. The effective date of benefit is 90 days from the first day of full-time employment.
Full-time staff members are eligible for Holiday Pay for all Aspire recognized holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving (Friday), Christmas Eve, Christmas Day.
Staff members are provided jury leave pay with official court jury summons.
Aspire recognizes the need for time away from work in instances of personal loss. Should a death occur in the immediate family of a staff member, the staff member is eligible for time off according to Aspire’s bereavement leave schedule.
Employee Resource Guide
Various benefits including EAP, Identity Theft, Financial Tools, State-specific online wills and legal library, and beneficiary support are available at no cost to employees and dependents.
Aspire Indiana encourages staff growth and development by providing appropriate orientation and in-service programs for all staff, as well as opportunities for participation in educational activities outside the Company. Types of Staff Development offered include: Orientation, In-service, web-based training, videos, outside educational programs, tuition reimbursement, collaboration with other mental health organizations, and other trainings as required.
Tuition reimbursement is available for qualified staff who meets all requirements which is reimbursed at $300 per semester, up to $600 per year.
